Why shoe sizing is five systems pretending to be one
There is no world shoe size. The US, the UK and continental Europe each built their own scale, at different times, from different starting units, and none of them agreed to line up. Japan and much of Asia skipped the whole argument and measure the foot in centimetres, which is the only system that describes an actual object rather than a position on a ladder.
The practical result is that the number printed in your shoe is a label, not a measurement. A US men's 9 and a UK 8.5 and an EU 42 are the same shoe wearing three different badges. Convert between them and you are translating labels, not remeasuring your foot.

US to UK and EU conversion chart
This is the men's ladder, and every row comes straight out of the shoe size converter on this site, so the chart and the tool cannot disagree. UK sits half a size below US men's. EU runs 33 above it. The women's column is the retail crossover, which is the rule shops use when you swap a men's shoe for a women's one.

Half sizes in EU are a courtesy, not a standard. Plenty of European makers only cut whole numbers, and when they do, the shoe you are handed for a 42.5 is usually a 43. If you are between sizes in EU, expect to go up.
The men's to women's crossover, and the trap inside it
There are two different women's conversions in circulation and they differ by half a size. Both are correct, in their own context, which is exactly why they cause so much trouble.
When you are swapping one finished shoe for another, the retail rule applies: a women's size is about 1.5 above the men's size for the same shoe. That is the column in the chart above, and it is what a shop assistant means when they say your men's 9 is a women's 10.5.
When you are starting from a foot length instead, the Brannock device, the metal slider in every shoe shop, puts women one full size above men, not 1.5. Anything that derives a size from a measured foot has to use that one, and our converter does.
So if a foot measurement and a shoe swap give you answers half a size apart, nothing is broken. You asked two different questions. Buy from the measured answer if the shoe is unfamiliar, and from the swap if you already know how that exact model fits you.
How to measure a foot properly
Stand up. A foot spreads under load, and a foot measured sitting down will read short by enough to matter. Put your heel against a wall, put a sheet of paper under it, and mark the end of the longest toe, which is not always the big one.
Measure both feet and use the larger. Most people have a mismatch of a few millimetres and a noticeable minority are a half size apart. Measure at the end of the day, when your feet are at their largest, and wear the socks you actually intend to wear with the shoes.
No tape measure? A sheet of US Letter paper is 11 inches on the long edge and A4 is 29.7 cm, so either one gives you a ruler you already trust. A credit card is 85.6 mm on the long edge under ISO/IEC 7810, which is precise enough to check a measurement you are unsure of.
Where brands drift, and by how much
Conversion gets you to the right ladder. It cannot tell you that a particular maker cuts small. Some drift is well documented and worth knowing before you order.
Converse is the standard example: Chuck Taylors run about half a size large, which is why the advice to size down is so widespread. That is a fact about the shoe, not about the conversion, and our Converse chart handles it separately rather than bending the general ladder to fit one brand.
Kids sizing is its own problem, because the scale restarts. Little kid, big kid and youth are separate ladders with their own numbering, and a youth size converts into women's at a different offset again. If you are buying for a child who is nearly adult sized, the youth to women's crossover is usually the cheaper way to buy the same shoe.
